All Athletes must complete the following before trying out for an MVMS sports team.

All athletes need to have a physical completed and turned in before trying out. Physicals are mandatory every two years. Click here for the paperwork.

Verify that they are academically eligible to tryout out for the MVMS team.

Students and parents will need to sign and turn in a Sports Questionnaire. Click here for the paperwork.

Parents/guardians of students who attend Mountain View Middle School may visit the school by securing a Visitor's Pass from the main office. Other requests need to be approved by administration.
